MACLAREN CHILDREN'S CENTER AND CHILD WELFARE SERVICES CASE MANAGEMENT SYSTEM (CWS/CMS)

This is to provide regional staff with information relating to MacLaren Children's Center (MCC) implementation of CWS/CMS.  Below is what Regional SCSWs and CSWs can expect to find completed and updated by MCC staff on their cases in CWS/CMS for all children placed at MCC.

  • MCC staff will update the Health and Education Notebooks and then create a Health and Education Passport for each child.  This Passport will be updated with current information on the Service Provider, CHDP Exams, Medications and MCC School information and will be available to the CSW while the child is placed at MCC.  A final version will be created upon the child's leaving MCC and put in the gray Medical folder.

  • In the Case Management, Open Existing Document - Case Notebook, regional CSWs will find documents summarizing the MCC Assessment Center Intake, Interim and Discharge Conferences, as well as Residential Care weekly evaluations for each child.  MCC Assessment staff will generate the conference documents, with input from Health Services, Mental Health and County Office of Education (COE), to provide a comprehensive assessment of each child placed at MacLaren Children's Center.  MCC staff will create REMINDERS in CWS/CMS for each case to notify the CSW "when and where" the next conference will be held. 

  • The MCC Admissions Record (DCFS 502) information is recorded in the CASE ALERT box on the CASE ID Page by MCC staff.  MCC child visitations and restrictions information are also displayed there.

  • MCC staff create a CONTACT when the child arrives at MCC.  The initial interview, the participants, and a brief assessment of the minor's most critical care needs are recorded on the ASSOCIATED SERVICES Page.  All services provided by MCC staff are recorded here also.  In addition, Monitored Visits are recorded on the ASSOCIATED VISITS Page.

  • MCC Health Services staff create a CONTACT to record the initial Medical Exam and CHDP Exams.  Immunizations, Diagnosed Conditions, Prescribed Medications, Referrals, etc., are recorded in the Health Notebook.

  • MCC school information is recorded in the Education Notebook by the COE staff.  The school information consists of hours, progress, etc., while the child attended the MCC school.
